In-home Recreation Program

Consists of 12 personal weekly visits from either an In-home Recreation Coordinator or a trained volunteer. Both individuals will facilitate a variety of leisure-based activities within the comfort of your own home for up to two (2) hours.

Regardless of the delivery method chosen, an In-home Recreation Coordinator will promptly connect with the client and care partner to complete a recreation and leisure assessment, which will help develop a holistic approach to implementing recreation and leisure pursuits. Once the 12 visits have been completed, an In-home Recreation coordinator will also provide support to the client and care partner with leisure education opportunities, and/or additional recreation opportunities within Alzheimer’s Society Southwest Partners.
