Counselling for the Care Partner

The Alzheimer Society Southwest Partners Social Work & Counselling Program has been developed to meet the needs of people whose lives have been affected by Alzheimer’s and other dementias.

Our Registered Social Workers provide in-office, in-home assessments, virtual or telephone assessments and work with all those involved to develop an individualized response to their individual needs.

Our Social Work services include:

  •  Individual and family counselling
  • Case conferencing
  • Advocacy
  • Information about and referrals to community resources
  • Support Groups
  • Memory Screening
